Insurance Process

• Payment in full for services provided at MARR is the sole responsibility of the client.
  
  
• MARR is out of network for all insurance companies and does not accept insurance.
  
  
• Some insurance companies will pay out of network benefits. If this is the case, it is the insured's responsibility to verify insurance benefits and complete the MARR Insurance Benefits Verification Form and attach a copy of the front and back of their current insurance card, prior to admission.

• As a courtesy, MARR will complete any pre-certification and/or utilization reviews with the insurance company, if required.
  
  
• In addition, MARR will mail completed claim information monthly to the insured or the appointed personal representative to file with their insurance company.
  
  
• It is the insured's responsibility to file claims and follow-up with their insurance company.
  
  
• Furthermore, it is the insured's responsibility to re-file and/or appeal, if requested by their insurance company.
  
  
• MARR is not responsible for any failure of an insurance company or managed care organization to provide reimbursement for any services provided. Please note that payment to MARR is the client's responsibility regardless of insurance status.
